Representation request form

We’re now asking members who need representation at a meeting to fill out this form. Once completed, the form needs to be sent via email to info@accordhq.org – once the form has been received, a casework officer will be in touch within 24 hours to arrange representation.

New Reps Form

If you are a new Accord rep, please complete the New Reps Info form and return it to HQ. Download the form here.

Casework Report Form

You must complete a Casework Report form after attending a meeting on behalf of a member. Details on how to return it are detailed at the bottom of the form. You can download the Casework Report form (PDF) here or Word Document version here.

Update Details Form

Use this form to get the most up-to-date information from your members including personal details, Accord’s data exchange permission as well as direct debit information. You can download a copy here. 

Recruitment Goodies Request Form

Recruitment goodies should only be used for recruitment purposes and to raise awareness of Accord. If you would like to place an order for recruitment goodies – please use our request order form. (Only those with Microsoft Word 2007 or above will be able to use this)

Those with an older version of Word (97 – 2003) should use this request order form (Print it out, fill it in and send it back to us)

Data Exchange Form

Accord is asking all members’ permission to exchange data with Lloyds Banking Group so that we are kept up to date with contact details, hours of work, workplace etc. If permission is given, it will save members having to notify Accord of any future changes. We require every Accord member to fill out the data exchange form they haven’t already done so. To download the form, click here.

Direct Debit Mandate

We are asking all Accord members who are currently paying their subscriptions via payroll to convert to direct debit as salary deduction will not be available for much longer. Any members paying via salary deduction should fill out a direct debit mandate and send it back to Accord HQ. To download the direct debit mandate, click here.
